RN CLINICAL SUPERVISOR - Full Time Position
Serves as a supervisor of home health and hospice services, assisting the Clinical Director in the coordination, development, evaluation and supervision of both programs. Working alongside the Clinical Director, this position assists in maintaining compliance of the home health and hospice programs with all applicable standards, regulations and requirements. Functions as liaison between hospitals, clinics, and the community for patients receiving services. HUMAN RESOURCE MANAGER - Full Time Position
Responsible for HR tasks. Oversees benefit administration process for all staff as well as medical leave process. Ability to collect, handle and maintain highly confidential information.  Full details are available in the job description.


MEDICAL SOCIAL WORKER - Full Time Position - Oregon License Required (LMSW, LCSW, CSWA)
Provides comprehensive psychosocial services and supportive counseling for hospice and home health patients, families, and caregivers. Core member of the Hospice Interdisciplinary Team (lDT).  Full details are available in the job description.


PHYSICAL THERAPIST - Full Time Position - Oregon License Required (Ask us about a $5,000 sign-on bonus)
Performs physical therapy services for patients requiring Home Health or Hospice care, under the care of a physician, according to accepted standards of practice.  Full details are available in the job description.


PHYSICAL THERAPY ASSISTANT - Hourly/Non Exempt - Oregon License Required
Performs physical therapy services for patients requiring Home Health or Hospice care, under the care of a physician and under the supervision of a Physical Therapist (PT), according to accepted standards of practice. . Full details are available in the job description.


WEEKEND RN - Hourly/Non Exempt - Oregon License Required
The weekend registered nurse (RN) is responsible for responding to clinical and non‐clinical situations for both Home Health and Hospice patients and represents the organization after normal business hours. The RN is responsible for providing skilled nursing assessments, interventions and supportive care for Home Health or Hospice patients and their families in accordance with the home health or hospice plan of care.  Full details are available in the job description.


LICENSED PRACTICAL NURSE - Full Time Position/Non Exempt - Oregon License Required
Administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. HOSPICE RN CASE MANAGER - Full Time Position/Non Exempt - Oregon License Required

Coastal Home Health & Hospice contracts with licensed massage therapists, speech therapists, and physical therapists to provide patient services, and for other professional services. Independent contractors must already be licensed and insured in the State of Oregon prior to contracting with us.
